As the largest prescriber of natural desiccated thyroid (NDT) in the United States, I have seen firsthand what happens when patients switch between different NDT formulations - and it is not always pretty.
The Phone Call That Changed Everything
Three years ago, I got a frantic call from a patient who had been perfectly stable on Armour Thyroid for two years. Suddenly, her symptoms came roaring back: crushing fatigue, brain fog, weight gain, hair loss. The works.
We ran labs. Her TSH had shot up to 12. Nothing else in her life had changed - same dose, same timing, same diet. What happened?
The manufacturer had changed their formulation.
Why NDT Quality Is Not Just About T3 and T4
When you see "65 mg thyroid USP" on a bottle, you are getting a snapshot of potency - but not the full picture. Here is what most patients (and many doctors) do not realize:

Most consistent formulation in my experience. The 2020 formula change caused some bumps, but they have stabilized. Good batch-to-batch consistency. My go-to for most patients.
NP Thyroid
Had major supply issues 2018-2020, but recent batches have been solid. Gluten-free formulation. Second choice for many of my patients.
WP Thyroid
Hypoallergenic formulation is clutch for patients with sensitivities. Smaller company, sometimes harder to source, but quality is excellent when available.
Nature-Throid
Currently on FDA recall list. I am not prescribing it until they resolve manufacturing issues.
Red Flags: When Your NDT Might Be the Problem
If you are experiencing any of these, it might be your thyroid medication, not your thyroid:
- Symptoms returned after years of stability
- Same dose, but labs are suddenly off
- Pharmacy switched you to a different manufacturer without telling you
- Pills look different than usual (color, shape, smell)
- Tablets are crumbly or poorly formed
What To Do If You Suspect Quality Issues
- Do not panic. This is fixable.
- Check your bottle. Note the manufacturer and NDC number.
- Contact your doctor. Get labs done to confirm.
- Ask your pharmacy if they recently switched suppliers.
- Request a specific brand. Your prescription can specify "Armour Thyroid only" or similar.
The Bottom Line
NDT is an excellent treatment option for many hypothyroid patients - when you get a quality product from a reliable manufacturer with consistent formulation.
